Personal Data Usage Regulations

In response to the "Personal Information Protection Management System" of National Chung Cheng University, we revised the "Membership Application Form" and added the "Application Form for Personal Information Service" in February 2017. Please read the notice in below:

1. For new members, please fill out the "Application Form of 2022 Friends of CCU (PDF) " and sign the "Consent Form for Provision of Personal Information" and submit it to the Association.
2. Family members and sponsor application form please download from this link (PDF).
3. Application for receiving or accessing member information, please fill out the "Application Form for Usage of Personal Information Service of Friends of CCU"and sign the "Confidentiality Agreement", and submit them to the Media Relation Division of National Chung Cheng University (CCU) in paper copy or through fax for review and approval. Without application, members are not allowed to use or access personal information collected by the council without approval.
